📋 **Requesting Your Order:** Please use the form below to request your prescription refill or food item. This convenient method will expedite your order pickup process. However, it's crucial to follow these guidelines:

-Order Processing: Kindly allow up to 2 full business days for us to process your order.

- Confirmation: Do not come to the hospital until you have received confirmation to pick up your order. A staff member will contact you by phone or email to confirm your order.

⚠️ Important Note: Keep in mind that for some prescriptions, an examination of your pet is required before refilling. This ensures your pet's health and safety, especially concerning potential side effects and the medication's suitability for your pet's current condition.

🩺 Pet Examination: Many prescriptions necessitate an examination before dispensing. This form is for pre-approved re-fills only.

📦 Special Orders: If your order is unique or specialized, we'll send you an email or phone you with an estimated delivery date to the hospital. Please be aware that special orders may take between 5 to 7 business days to arrive at the hospital.
